Re: Notice of Intent to Establish a Home Education Program

Dear Superintendent,

In accordance with Florida Statute 1002.41, I am writing to notify you of my intent to establish a home education program for my child(ren) listed below:

This letter serves as the required written notice to the county school district superintendent.

Pursuant to Section 1002.41, Florida Statutes, the parent shall notify the district school superintendent of the county in which the parent resides of her or his intent to establish and maintain a home education program. The notice shall be filed in the district school superintendent's office within 30 days of the establishment of the home education program.

This notice is provided in accordance with Florida Statute 1002.41.

I understand my obligations under Florida law, including the requirement to maintain a portfolio of educational records and materials, and to provide an annual educational evaluation for each child. I will preserve the portfolio and evaluation results for two years. Please acknowledge receipt of this notice at your earliest convenience.
